Montefiore's Commitment to the Community

 

Primary Care and Outreach Programs for the Disadvantaged


During the past two decades, Montefiore has greatly expanded its ambulatory care facilities. High-quality medical care is now available in virtually every neighborhood of the Bronx and in nearby Westchester through the Montefiore Medical Group, an extensive network of physician offices and community health centers. On its Bronx campuses, Montefiore offers primary and specialty care services at its Family Care Center and at two faculty practice centers - the Jerome L. and Dawn Greene Medical Arts Pavilion at the Moses Division and the Montefiore Medical Park on Eastchester Road. Montefiore's North Division offers a family health center and ambulatory mental health and substance abuse services at sites nearby, as well as four multi-specialty private practices in the northeast Bronx.

Montefiore is located in the Bronx, New York City - one of the country's most underserved communities - and is proud of its 120-year tradition of serving the needs of the traditionally medically underserved. Today, the medical center continues this legacy of working with the disadvantaged by employing modern disease management concepts, such as "evidence-based medicine" and "population health," and using innovative computerized clinical information systems. Leveraging these collective patient data, Montefiore uses new software called Clinical Looking Glass to conduct sophisticated studies aimed at improving the health of entire patient populations.

The beneficial network of community programs that Montefiore has established for the citizenry of New York City and Westchester County includes:

1. A mobile health program, operated in conjunction with the New York Children's Health Project, that serves homeless children and families in various locations throughout New York City;

2. Outreach programs providing health services to homeless persons in soup kitchens and shelters throughout the Bronx;

3. Leadership in developing a senior house calls program to meet the health needs of the elderly in their homes;

4. A nationally recognized lead poisoning prevention program for screening and treatment of populations in the Bronx at highest risk for lead poisoning, and a model housing program, called Safe House, to shelter families of children being treated for lead poisoning while their dwellings are made lead free;

5. Bilingual/multicultural health services for the borough's diverse populations;

6. The Child Protection Center for counseling and treatment of abused children and their families, as well as similar services for elderly patients subject to abuse;

7. School health programs that in 2010 will serve more than 15,000 children at 16 elementary, middle and high schools, at the vanguard of addressing childhood obesity, asthma and diabetes in the Bronx;

8. A series of health careers education programs in Bronx high schools;

9. Innovative programs of HIV testing, counseling and medical care for enrollees in its methadone treatment program;

10. Outreach and prenatal/child care services for pregnant women who are either HIV-infected or at risk for HIV infection and an innovative volunteer program offering a full range of support services for HIV-infected patients and their families.

We treat the most fragile patients in Montefiore’s medical, surgical, neurological and cardiothoracic intensive care units, but critical care at Montefiore also extends beyond those areas. Our unique "ICU Without Walls" service is a rapid response team of intensivists and respiratory therapists who are on call 24/7 to evaluate and treat patients in need of critical care in other areas of the hospital.
