
The Clinical Diabetes Center at Montefiore Medical Center provides education and support to patients with diabetes and is dedicated to helping them live the healthiest and fullest lives possible. With the instance of diabetes on the rise at an alarming rate in the U.S, we are a leader in research to expand available treatment options.
Some 4,000 patients are treated at the Clinical Diabetes Center annually. Montefiore is among a handful of diabetes centers that have earned the prestigious American Diabetes Association recognition award, given for patient satisfaction.
The majority of our team of experienced nurses, nutritionists and endocrinologists are board-certified diabetes educators. We are trained to share our specialized knowledge of diabetes treatment with those receiving both inpatient and outpatient care at Montefiore.
We serve adult patients referred by their primary care provider for education and who receive treatment through an insulin pump. Our aim is to empower diabetic patients through knowledge and help them improve their health through better management of diet and medication. Our nutritionists teach classes that offer a foundation in how diet is related to a patient’s condition.
Although we do not offer primary care to our diabetes patients, we provide access to expert care through individual appointments with our diabetes specialists. These experts are all on the faculty of the Albert Einstein College of Medicine, and at the forefront of diabetes-related research and treatment techniques. We are able to offer individualized care to patients with special health considerations by referring them to other expert doctors within Montefiore, such as the focused care we offer diabetic women who are pregnant.
The Albert Einstein College of Medicine's Diabetes Research & Training Center (DRTC) is one of only five centers in the country to be awarded a competitive grant from the National Institute of Health (NIH) to fund ongoing clinical and transitional research. We work very closely with the Albert Einstein College of Medicine to develop innovative approaches to diabetes treatment and we participate in clinical trials of new diabetes medications. In addition, Montefiore has been involved in studies of heart disease for patients with type 2 diabetes and of diabetes prevention.
We know that it is difficult to stay upbeat and diligent when managing a chronic condition. Our support groups meet regularly on both campuses to help patients remain attentive to caring for their disease and maintaining their overall health. The PROMISED Program (Proactive Managed Information System for Education in Diabetes) provides 10 hours of patient education and patient empowerment to persons with type 2 diabetes. The program was designed to work closely with primary care physicians, and has been shown to improve clinical outcomes in more than 1,000 patients. For more information please call 718-920-7247.
We are extremely committed to serving our immediate community, which includes a large Hispanic population. We are currently working with The National Alliance for Hispanic Health. We also have an ongoing collaboration with the Hispanic Center of Excellence at the Albert Einstein College of Medicine, which is the first such center in the state.
