Long Term Care

Our Long Term Home Health Care Program provides professional care at home to the chronically ill or disabled and to the frail elderly who might otherwise require nursing home placement. This program is also known as "The Nursing Home without Walls." The LTHHCP is a New York State Medicaid funded program available to people who are eligible. LTHHCP services include:

• Registered nurses who provide health assessments and education about disease management and medication regiments
• Social work services
• Personal care workers
• Home health aides
• Physical, occupational and speech therapy
• Medical supplies, equipment assessment and safety evaluations
• Registered dietitians and nutritionists
• Laboratory services
• 24 Hour on call coverage
• Personal emergency response systems
• Respite care
• Medical transportation services
