Certified Home Health Care Agency(CHHA)
The Certified Home Health Care Agency, is our short-term professional home healthcare program which assists patients in the transition from the hospital or nursing facility to home, with a focus of improving health and independence. Extended services are available to community members who require a skilled level of care.
The focus of the CHHA Program is to:
• Provide short term skilled health care
• Instruct and educate about medications
• Instruct and educate about management of disease
• Ensure safety in the home environment
